Raising the Bar of Pediatric Patient Safety: Report From the Institute of Medicine

      shocked and stimulated many health care providers. The numbers of patients potentially affected by the errors in medicine were staggering. However, potential approaches to correcting this problem seemed close at hand and, with the dissemination of those reports, likely to have their implementation catalyzed by health care industry stakeholders. At a national level, these reports received an enormous amount of attention from the media and from the federal government's provision of additional funding for the Agency for Healthcare Research and Quality (AHRQ) among others to investigate and determine approaches to alleviating the problem of medication errors in the health care industry. This agency developed a program of grants to demonstrate the value of information technology and other approaches to patient safety. One barrier that quickly became apparent was the critical need for information exchange standards to allow collection of data that would be externally meaningful and potentially able to be aggregated with data from other sites.
