I was recently asked to speak about a mistake I have made in my career. I was overwhelmed
at the number of memories that came to mind. Somehow my errors are seared into my
brain and the successes seemed to have disappeared as though they had never happened.
Despite many vivid memories of when things went wrong, 1 patient encounter felt the
